I'll second the mention of bad TL074 opamps. I found that a machine I fixed for a friend.
I don't think the LM386 had failed, though I may have replaced it as a precautionary measure.
The TL074s are easy to test, if they're working correctly then the + and - inputs should be at a similar voltage. Usually when they fail, the output sticks at full positive or full negative supply.
