Can’t answer all your questions, but ROM emulators are not new.
Some EPROM programmers could use an adapter to provide a ROM during development by using their built-in RAM to hold the data. Similarly there were other specialised devices that could something similar, but these often cost a fair bit of money.
I’m sure there were DIY designs that bank switched between a ‘boot loader’ ROM and SRAM. The ROM containing just enough code to load the ROM image into RAM, then you would reboot with the SRAM in use.
